Branded vs generic GLP-1 medications in India: what patients should know

Updated: Oct 1
First separate the molecule from the name on the box
Semaglutide is a molecule. Wegovy and Ozempic are brand names associated with semaglutide in different indications and dosing contexts. As patents expire and more manufacturers enter the Indian market, patients will increasingly encounter alternative semaglutide brands.
The first question should therefore be: what active medicine is this?
What an established brand gives clinicians
Long-established innovator products generally come with large pivotal clinical-trial programs, extensive product information and a well-characterised safety profile. That evidence is valuable because clinicians understand the doses studied, the expected adverse effects and the outcomes observed in defined patient populations. However, the existence of an established brand does not make every other approved product clinically inferior.
What a legitimate generic should demonstrate
An approved generic is expected to meet applicable regulatory requirements for quality and equivalence. The exact requirements depend on the medicine and formulation, so patients should not try to judge quality from price or packaging alone.
Equally, products described loosely online as 'generic', 'compounded' or 'research' should not be assumed to have the same regulatory status.
Why supply chain matters
WHO has documented falsified semaglutide products internationally. A trustworthy pathway should allow the medicine to be traced through authorised channels and stored according to the product requirements.
An injectable product is not just an active ingredient; device quality, concentration, storage, handling and labelling also matter.
How we would explain the choice to a patient
The medicine should be selected because its evidence, approval, formulation and clinical profile fit the patient, not simply because it is the most famous or the cheapest.
